Same Defect, Same Line, Third Time This Year. The Fix Has Not Been Documented.

Same defect. Same line. Third time this year. The first time it happened, someone fixed it. The second time, someone fixed it again. Now it is back. The fix is not working, because the fix has never been put in the standard.

Pain: Same defect, same line, third time this year.

When the same problem returns, it means the fix is happening at the symptom level, not the cause. Someone corrects the output. The line moves on. But the process that produced the defect has not changed. Nothing has been written down. Nothing has been updated. The next operator to run that job inherits the same gap. This is process consistency risk: when output quality depends on who is running the job rather than on what the standard requires.

The reason defects repeat is rarely that your team does not care. It is that the correction never becomes part of the official method. The knowledge of what went wrong and what the right approach is stays in the head of the person who fixed it. When they are absent, or when a new operator runs the job, the defect recurs.

Fix: Document the fix in the standard.

The next time a recurring defect is resolved, do not move on until the standard has been updated. Write down what went wrong, what the correct step or setting or check is, and where in the process it belongs. Update the job card, the SOP, or whatever written reference your team uses. Brief the relevant operators on the change.
